Dry needling is the use of small needles to de-activate myofascial trigger points.  Per Travell and Simons, Myofascial trigger points are hyperirritable spots, usually within a taut band of skeletal muscle or in the muscle's fascia.  The spot is painful on compression and can give rise to characteristic referred pain, tenderness, and autonomic phenomena generating a myriad of complex musculoskeletal and orthopedic conditions.  Dry needling trigger point therapy is most effective when a local twitch response is elicited as it inhibits abnormal end plate noise and alters the chemical milieu of a myofascial trigger point.

Trigger point dry needling is an appropriate adjunctive treatment for all musculoskeletal dysfunctions as it serves to diminish neuropathic supersensitivity by reducing the nocioceptive compression that leads to pain and shortened sarcomeres. Patients do commonly report an immediate pain reduction and/or elimination after eliciting a local twitch response but may experience soreness for one to two days post-treatment.
